Job Description
The Program Manager, Maryland Primary Care Program is responsible for the ongoing execution of the Maryland Primary Care Program (MDPCP) at Privia. This includes holding total ownership and accountability of program requirements and partnering with clinical leadership and internal teams to manage program performance. Additionally, this role is responsible for maintaining close relationships with the Maryland Department of Health and CMMI contacts to ensure Privia is positioned for success given any future program changes. This role also serves as the main Privia liaison to Chesapeake Regional Information System for our Patients (CRISP), Maryland's Healthcare Information Exchange. The ultimate goal for this individual is to oversee Privia’s success in the Maryland Primary Care Program.
- Own and hold accountability for the successful day-to-day operations of the Maryland Primary Care Program (MDPCP), including quarterly funds flow, and annual/semi-annual program requirements (i.e. eCQM reporting, financial reporting, care management reporting)
- In collaboration with the Analytics and Product Teams, manage internal and external reporting needs related to the MDPCP and CRISP
- Ensure successful completion of annual Patient Family Advisory Council (PFAC) meetings at each MDPCP practice
- Serve as Privia's CRISP liaison, meeting regularly with CRISP and overseeing recurring processes and ad-hoc issues
- Develop ongoing and ad hoc internal and external communications and presentation materials related to MDPCP and CRISP
- Troubleshoot MDPCP and CRISP issues
- Maintain accurate provider rosters
- Monitor program changes and present them succinctly to various stakeholders
- Regularly review data from multiple sources, identifying key trends and action steps
- Collaborate closely with the Population Health Team, Care Management Team, and Operations Consultants to align priorities and ensure successful operation of the MDPCP program
- Support the Care Center Transformation team in preparing for POD meetings and Clinical Leadership Summits as it relates to the MDPCP program
- Serve as an MDPCP/CRISP subject matter expert across internal and external teams, contributing to cross-team projects and leading relevant discussions
Qualifications
- Bachelor's degree required
- At least 5 years of progressive experience in a fast-paced healthcare environment
- Value-based care knowledge, experience with CMS programs is a plus
- Experience and comfortability working with intricate financial data and foundational Excel formulas and pivot tables
- Extremely organized and self-sufficient in tracking key deadlines
- Experience leading cross-team projects and managing multiple priorities
- Experience working in an EMR
- Strong attention to detail and ability to interpret and explain complex data
- Experience with project management tools
- Strong communication skills with experience presenting clearly to stakeholders at multiple levels
- Proactive and a self-starter
- Must comply with HIPAA rules and regulations
The salary range for this role is $71,000.00 - $84,000.00 in base pay and exclusive of any bonuses or benefits (medical, dental, vision, life, and pet insurance, 401K, paid time off, and other wellness programs). This role is also eligible for an annual bonus targeted at 15% and restricted stock units. The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.
